Fluid accumulation in the scrotum is a condition in boys and men, which, depending on its nature, may be called hydrocele or spermatocele.

A hydrocele is an accumulation of fluid in the sac around the testicle. A spermatocele is an accumulation of fluid around the vas deferens above the testicle.

If fluid build-up is so great that it causes discomfort, especially while sitting, lying, having sexual relations or doing a particular type of work, surgery – hydrocelectomy – is recommended.

The patient can go home and take on activities which are not physically demanding on the same day after the surgery. Those who do manual labour can go back to work about 3 weeks after the surgery. Sexual relations can be resumed 4 weeks after the surgery.

We close the incision with dissolvable thread, therefore you will not need to return for suture removal.

What should you know about a hydrocelectomy procedure?

  • a complete blood count;
  • blood coagulation test (APTT);
  • electrocardiogram (EKG) and its evaluation;
  • blood glucose level test;
  • other tests if you have chronic diseases, as prescribed by your doctor.

The patient can bring their test records from other healthcare institutions, or they may have them done at the Medical Diagnostic and Treatment Centre. The tests take 2 hours. The tests must be performed no earlier than 14 days before the surgery.

It is a minimally invasive procedure. A small incision is made on the side of the scrotum during the surgery. The hydrocele sac is opened, the fluid is drained, the sac lining excised and what remains is inverted and stitched in a way that any excess fluid would be immediately drained by the lymphatic system. This is important to prevent a hydrocele from recurring. 

These surgeries are usually performed under local anaesthesia,thus, the patient can eat, drink, and walk soon after the surgery. The surgical wound must be dressed everyday for 7 days after the surgery. The patient can go home within 24 hours. The skin of the scrotum is usually sutured with dissolvable thread, which, consequently, does not need to be removed as it falls out in 10-14 days.
Sexual activity, intense physical activity, and scrotum injuries should be avoided for the first month after the surgery, because it may cause bleeding.

A recurrent hydrocele—if the lining of the sac is not thoroughly removed, its residue can produce fluid again.

Bleeding—may occur during the surgery or immediately after it, especially if the patient has blood coagulation defects. Later post-operative bleeding at the site of the incision can occur if the scrotum is injured.

Some other (rare) complications are also possible, especially if the patient has other conditions.
